Campbell Little League Parent Code of Conduct

Campbell Little League has implemented the following Sport Parent/Guardian Code of Conduct for the important message it holds about the proper role of parents/guardian in supporting their child in sports. Parents/Guardians should read, understand and agree prior to their children participating in our league.

Participation Terms and Conditions
Approval for player to participate, acknowledgement of potential injury, acknowledgement verification documentation is needed, acknowledgement skill evaluations may be needed.

Medical Release Form
Provides player's guardian contact information, physician information, allergies/medical. Team manager will have at all games.

CDC Youth Sports Concussion Information
Fact sheet for parents with information to help protect athletes from concussion or other serious brain injury.

Media Release Form (optional)
Gives permission to Campbell Little League to use photographs, voice recordings, or video taken during the games and events to help promote the league.
